To mail in your completed forms, use certified bonded carrier delivery for mailed applications.
(Fed-Ex, UPS, etc.). There are two forms available on the links above, one for Ecuador and one for
the remaining countries. Complete both and make three copies. Mail original and two copies to: Asst.

Attn: CDSOA Team, 6650 Telecom Drive, Suite 100, Indianapolis, IN 46278.
You can also fill out the forms online at http://www.pay.gov and submit them electronically on that site.
You will have to set up a login and password first on that site if you have not already done so in the past.
Keep one copy for your records. PLEASE DO NOT MAIL APPLICATION TO LSA.
Applications need to be received by US Customs by August 1, 2016. Use the applications provided on
LSA's website as explained above or complete applications on-line at the pay.gov website as explained
above and on pages 34626 & 32627 of the Federal Register Notice: Federal Register / Vol. 81, No. 104 / Tuesday, May 31, 2016 / Notices. Please read the Federal Register Notice. It contains information on how to complete and submit your application.
